Medical Information: If
you have a question about a particular medical service and availability, please
contact the General Leonard Wood Army Community Hospital (GLWACH) EFMP Special
Needs Advisor / Case Manager at 573-596-0049.
Upon arriving at Fort Leonard Wood, check in
with TRICARE and to get a Primary Care Manager (PCM) assigned. Your PCM must
refer you for medical care that is outside of GLWACH and approved by TRICARE.
Also, visit the GLWACH EFMP Special Needs Advisor / Case Coordinator to update your exceptional
family member information.
Housing: If you wish to request an exception to policy for immediate or
priority housing, supporting documents from your EFM physician, your command,
and MEDDAC Form 9 will be required. Requests are reviewed by hospital staff and
the hospital commander makes recommendations to the Housing Office. Thereafter,
a decision is determined based on your situation. You can request a copy of
MEDDAC form 9 from the GLWACH SNA/CC to have a completed by a current physician
and send it forward for processing prior to your arrival.
Child Care: The Fort Leonard Wood Child Development Center and Family
Child Care homes are two options available for children. School Age Services is
available for grades kindergarten through- fifth, and the Dru Pippin Youth
Center is available for grades sixth through eighth. All registering children
are screened for special needs and children who identified with special needs
are scheduled for a Special Needs Resource Team (SNRT) meeting to identify any
program modifications to provide quality child care. Contact the Central
Registration Office at telephone 573-596-0238 for mandatory registration for
child care and youth sports. The Central Registration Office is located in Room
1111, Building 470, Community Service Center.
Educational/Recreational Programming: The EFMP offers a support group
called Parents as Leaders (PALS) that provides support and addresses EFM issues.
The EFMP also sponsors special needs youth sports programs and coordinates with
community organizations to offer special needs camps.
