| Mission |
| Provides tailored integrated support services while serving as an information broker/clearing house in a location proximate to Warriors in Transition (WT) and their Families. The purpose of the services is to equip and aid Warriors in making life changing decisions as they transition either back to duty or to civilian life. |  |

| - The Soldier and Family Assistance Center strives to provide a supportive environment for Soldiers and Family Members as they deal with the stress and uncertainty of injury and the effects of war.

- The SFAC conducts referral, liaison and coordination with installation and local agencies to deliver non-medical services to Warriors in Transition and Family Members in support of the Army Medical Action Plan. The SFAC provides services on-site at the SFAC facility, brings forward services on a part-time basis, or establishes priority appointments on behalf of Warriors in Transition at the office.

- These delivery methods along with caring professional staff ensure Warriors and their Families are taken care of whether they are transitioning to civilian life or are returning back to duty in another unit.

- The Fort Leonard Wood Soldier and Family Assistance Center (SFAC) is intended to be a center for Warriors in Transition (WT’s), Department of Defense (DoD) personnel, and their Families to gather for comradeship, to share information, and to provide tailored, integrated support services in a warm, friendly environment.
- The SFAC is designed to provide direct support to Warriors in Transition and their Family Members while assigned to the Fort Leonard Wood Warrior in Transition Unit (WTU). The Soldier and Family Assistance Center is partnered with the WTU to provide the full range of services and is located at 140 Replacement Ave (Bldg 470) in rooms 2125 and 2202.

- The SFAC provides coordination with installation support services to ensure that the WT has priority placement for all housing, rehabilitation, career development, education and any other services needed to expeditiously assist the WT in transforming back to active duty or in transitioning from the Army. All support services will be provided on site at the SFAC facility, brought forward on a part-time, as needed basis or established as priority appointments on behalf of WT’s at other offices.

- The SFAC Director provides coordination with installation support services to ensure that the WT has priority placement for all housing, rehabilitation, career development, education and any other services needed to expeditiously assist the WT.

- Information Referral and Follow Up Coordinator
|
- The information and referral coordinator will be our first point of contact at the SFAC. They’ll greet you, provide you information on Fort Leonard Wood and the local area, and direct you to the correct service providers if you need support.

- Education Services Specialist
|
- The education specialist assists WT’s and their family members with information and guidance regarding education benefits and education related programs. Counseling includes, assisting with GoArmyEd online, researching college programs, assistance with choosing a degree program, and financial aid information.

- Human Resources Specialist
|
- The Human Resources Specialist will help Soldiers: complete DD Form 93/SGLI; update Enlisted Records Brief (ERB) /Officer Records Brief (ORB); schedule priority appointments for WT’s/issue Common Access Cards (CAC) and identification cards to eligible Family members; coordinate entitlements and services with local, regional, state and other federal agencies to include the Department of Veterans Affairs; inform and provide WT’s separating from military service, appropriate transition services and decision support assistance; complete/front load DD Form 214 worksheets for WT’s as needed. coordinate and ensure WT’s are provided timely Orders Processing within five duty days of notification; assist and coordinate with Retirement Services IAW AR 600-8-7

- The Social Services Coordinator assists WT’s through crisis intervention, resolution focused consultation, substance abuse prevention education/intervention, stress management and suicide prevention program development
